outlook 2003 business note

When I want to create on new business note for account:
1) the print out entry"Phone Call".
2) Start end end are wrong. I checked the date in time on control panel and
it is okay.
How can I correct those problems? I am using outlook 2003 business contact
manager and it is not shared.
