Outlook 2003 calendar with exchange 2003 updated and canceled meet

Hi,
I have a user of exchange 2003 with outlook 2003.
Some times when he gets an update to a meeting or when a meeting is canceled,
the meeting won't change or be deleted from the calendar.
Instead appears an additional identical meeting with the word: canceled or
updated.

What can I do to fix this and prevent reoccurrence?
