Outlook 2003 contacts view problem

The contact view in Outlook 2003 is not correct. When you open a contact and
look under the General Tab instead of seeing:

Full Name
Job Title
Company
Files as

Phone Numbers
Business
HOme
Business Fax
Mobile


Addresses
Business

You see...

Last (G):__someone's last name__ First (M): someone's first name
Company: some comapny name

Department
Job
Files As

Phone Numbers
Business
Home
Business Fax
Mobile

Addresses

Business
Zipcode (Z):
State (D):
City (O):
Streeet (B):
Country (N):

I would think this is a view setting but I can't find it. Can you tell me
how to set it back to defaults?

Did you create a custom form? Do you have Business contact manager
installed?

Can you post a screenshot online so we can see it? If you need a place to
post it, xsolive.com has an outlook screenshot library.

Hi,

No, I did not create a custom form.

this is how my contact look like now...

Ok. It's definitely a custom form. Do you use any add-ins that might affect
contacts? Right click on the contacts folder, choose properties - the
General tab will have a box for 'use this form when posting to this
folder' - what is the name of the form? IPM.Contact is the default form.

Yes, the "when posting to this folder, use:" is IPM.Contact.

I did not add any add-ins. I tried system restore on 29/08/2008 and it was ok,
however it went back on 06/09/2008. when I click on system restore again I
notice that "printer driver microsoft office document image writer installed"
was installed on 06/09/2009, 2330hr.

That's the correct form for Outlook. Are you using Business Contacts
Manager?

Until someone figured out what installed that form, possibly not. You could
try deleting the forms cache, but if the form is published, it will be
recreated when you make a new contact. If that form is not used when you
make a new contact, use docmessageclass - see
http://www.outlook-tips.net/archives/2004/20040525.htm - to change the form
the existing contacts use.

If you are using a PST, make a new profile (can be no mail or use a fake
mail server) - do you get the same contact form in the new profile?

I've tried repaired, and also uninstall and reinstall again. It still the
same. What could be the problem here?

Is there any solution for me?

Just the other day, the form for new contact was just as described in the first post, but then next day, it changed.
It is annoying and I did not install anything to cause any glitches or changes.

Any help?
