Outlook 2003 does not track all delivery receipts

I automatically send internal emails using delivery receipts but they do not
always show up in the tracking. The delivery receipt comes in with Subject:
Delivery Status Notification(Success) instead of Subject:(the subject name).
I think Outlook is unable to "tie" the delivery receipt with the original
sent item so there is no tracking available on the received delivery
receipts. I can send emails to the same users using email with or without
attachments, but however when sending email with a specific Word attachment,
the delivery receipts do not come in with correct subject name.
