Outlook 2003 Form Attachment

Is there anyway possible to add an attachment to a form? Can the information
on a form be pulled directly from the form and added directly into a
database? The form will have two combo boxes, a text box, and an attachment
if possible.

If you want to be able to add attachments, you must leave the normal message box on the form.
