outlook 2003 Journal "entry type", start, and end don't take values.

I'm running O2003SP2 on windows 2000. My journal works as expected,
but the "entry type" field remains blank, no matter what I enter, and
the start and end fields contain "none", no matter what I enter on the
new entry form. The duration and created fields have their correct
values.

Why do these fields forget their values, and how can I fix it?
