Outlook 2003 not sending email no error

I'm having a strange problem with sending email messages from Outlook 2003.
I have my contacts setup so that they display as John Doe ([email protected]).
That is also the way that most emails look when I send them.

However, I've run into two instances when emails appear to send, but the
recipient never receives them. In each case, the behavior within outlook is
the same.

I type the recipient's name into the new email window. i.e. John Doe.
Instead of outlook finding a match and presenting it to me, it just leaves
the name sitting there. I then click the send button and instead of finding
a match and asking me if that is the correct one, it just puts the email into
the Outbox.

If I look in the outbox, the message recipient is presented as 'Doe, John'
exactly as I typed it. It has single quotes around it and puts the last name
first.

When I perform a send & receive, the email moves into the Sent Items folder,
but the recipient never receives it. I also never receive notification that
they never received it.

I've run the pst repair tool on the pst file, but that hasn't corrected
anything. Of all the emails I've sent only two people in my contact folder
have had this happen. So it is intermittentant.

How can I fix this?
