Outlook 2003: "Office 2003" Look Gone

I'm having a weird problem with Outlook (2003 SP3):
A few weeks ago I switched back and forth between XP themes, which at
some point caused some of the Office 2003 programs (I don't know it it
was all of them) to lose the 'Office 2003' (3-D) look. When I returned
to the "Classic" theme, they got the 3-D look back. Except for
Outlook. This still had the 'flat' look, and menus and items like
Office 2002 (or 2000, I don't remember exactly how the resp. versions
looked).
Some days later, the 'Office 2003' look including toolbars, menus and
icons suddenly came back.
Just do disappear again yesterday.

How can I fix that for good?

By conicidence, all my rules (about two dozen) started acting
strangely at about the same time (a few weeks ago). Sometimes they
work, sometimes they don't. Sometimes disabling and reenabling them
fixes them. Sometimes a relaunch of Outlook fixes them. Sometimes it
doesn't. Don't know whether there's a link between both problems.
