Outlook 2003. "One-off" behavior with custom forms on read page only

I have a particular custom form that displays the compose page correctly,
but when sent the arriving read page does not display the form but rather a
"One-Off" i.e. IPM.Note message. The forms cache has been cleared. It
should be noted the form in the user's Sent messages Folder is also
incorrect. Is here any way to correct this behavior?
