Outlook 2003-Questionnaire style form, reverts to email style on r

I have created a simple outlook form for a questionnaire to send out to
people. When testing, and replying (to complete the questionnaire) the form
style goes and is in the simple email format on 'reply' - what do I have to
do to ensure when a user replies, they will be able to return the form to the
sender in the same format?

In which context is this being send?
Are all the recipients using Outlook and are they internal to you and within
the same Exchange organization?
This would really affect the answer to your question.
Also, how did you create the form? Is there any programming behind it or
just interface?
