Outlook 2003, RPC/HTTP, no Login dialog

I have a customer mailbox hosted on Microsoft Exchange 2003 server with SP2.
The customer is using Outlook 2003 and Windows XP SP2. Configured to check
mail via RPC/HTTPS. The customer is not a member of the Exchange server
domain. When starting Outlook for the first time Outlook hangs at the
initial configuration and an icon for the login dialog box appears on the
taskbar but is not available to input username and password.
Another user in the same office with the same apparent configuration works
fine.
I've tried deleting and recreating the user's Outlook profile, repairing
Outlook, removing and reinstalling and fully patching.
outlook.exe /rpcdiag shows Outlook connecting, but never connected.
If I configure my laptop the same way and run outlook.exe /rpcdiag I see the
exact same progress in the RPCDiag screen until I enter my username and
password.
Any ideas why the login dialog box is not available?
