Outlook 2003 SP1 tools --> Choose Form..."Form load Failed" Why?

I folled directions on createing Outlook templates. This worked fine when I
saved etc.

But when I click on Tools --> Choose Form.... I get a error message box
stating

"Form load Failed" I cannot find any information on this problem in the KB


Thanks for any assistance.

Clicked on Inbox, then clicking on tools --> Choose Form....

I then get this error message.

OK, let me ask the question another way. Where did you publish the form? Or
did you save it as an .oft template file?

I am sorry,

I opened up a new e-mail. Filled in some information then went to

File --> Save As Oft file.

Anyway this error happens before anything comes up when picking
Tools --> Choose Form.

Do you get the same error if you double-click the .oft file?

No.

Just from the Tools --> Choose Form menu. This happens before even getting
a list of forms to choose.

I'd try resetting the menu bar -- View | Toolbars | Customize > Select Menu
Bar and click Reset. If that doesn't work, rename the Outcmd.dat file and
restart Outlook.

I'm experiencing the same problem. I tried the two suggestions-
resetting the menu bar and renaming the outcmd.dat- but the problem
persists.

Also, I couldn't find the outcmd.dat file by searching C:\ even with
search all subfolders. I hunted it down manually and found it under
Documents and Settings -> Application Data -> Microsoft -> Outlook.

I'm using Outlook 2003.
