Outlook attachment removed when reached recipient mailbox

I have a problem with Outlook. When a sender sent an email with attachment to
another outlook user, the recipient received the email without attachment.
When compare the email's header in recipient's Inbox, it shown recipient's
received the mail without attachment (X-MS-Has-Attach: <empty>). But from the
sender's sent item, it showed original mail with attachment. Can anyone
advise?
Enviroment: Exchange 2003 SP2, Outlook 2003 SP2.
