Outlook change password prompt

Hi,
I am using exchange 2003 and outlook 2003. Having a problem on the
update new password prompt when AD login/password is expired. On the
password prompt, it doesn't fill in the domain, username. It only
fills in the exchange server name. Does anyone know how to fix this
problem?
