Outlook checklist form

I have created an outlook checklist form where the user when submitting a
request for purchase can checklist all the necessary documents they are
sending so it's easy for both the person making the request as well as the
person receiving the request to know what has been sent and what is missing.
The form has two tabs. The first tab is almost exactly the same as a regular
message form, and second tab contains the checklist and a few other fields.

Here is the problem: When I send the outlook form to others some people only
see the first tab (The checklist tab is not visible to them). I have sent the
form to many people to test it, and it seems fine on some people's computers
but not on others.

I have checked their outlook version and that isn't an issue. Please advice
me on what might be going wrong so I can fix it. Thanks

How are you "sending the form"? What version of Outlook, including service pack level, do the problem recipients have?

I have sent the form in two ways. 1. Since its a message form you can send it
using the send button 2. Saving it as an oft file and sending them the file
as an attachment to view.

Everyone I have sent them the file have Outlook version 2003. I have SP2
service level. not sure what the others have. Could that be a problem?

You will not be able to use technique #2 with Outlook 2003 SP2. Ideally, you'll have published the form to the Organizational Forms library on your Exchange server, which will let you use technique #1.

On some of the computers it seems to work eventhough they also have SP2.
Before gettting it on the exchange server where i would need permission to do
so, is there any other way of sending the form as a test for people to view
and suggest how the content of the form can be improved. Does this have
anything to do with outlook setting? I still don't quite understand why it
works on some computers and not on others. They all have Outlook 2003 with SP2

Maybe the successful SP2 users are saving the .oft file to their hard drive, then running the saved file.

For best results, each user will need access to the published form definition. If you cannot publish to the Organizational Forms library, users will need to publish to their Personal Forms library. Send them the .oft file with instructions to save it to their hard drive, then run the saved file and publish it with the same name as you're using.
