Outlook - Contacts Question

I've been trying to figure out this problem for a few days now and I was wondering if you could offer any insight into the problem that I am experiencing. Here is the problem: I have created a custom form with 5 pages with additional information for each contact. This information was all accessible until about a week ago. I do not know where this information has disappeared to or how to make it reappear. I have tried changing the view that the contacts are in, but this does not work. The only pages that show up are General, Details, Activities, Certificates, and All Fields, but I had added P.2, P.3, P.4, P.5, and P.6. Is there anyway whatsoever to get these pages to reappear? Am I doing something wrong when trying to change the view? I do not know how to get this information back for all of the contacts and I am very frustrated. I have tried just about everything and I am about at wit's end. Can you advise me as to what I should do or where I can go to get more help?

Either in the All Fields tab of the contacts or in a folder view look at the
MessageClass of the contacts. You can add that field to a table view by
right-clicking on a column header and selecting the Field Chooser. In that
dialog select All Contacts fields from the drop-down and then MessageClass.

Do the contacts have your custom MessageClass? Is your custom form published
and if so where is it published? Have you tried clearing the forms cache?

If the items don't have your custom message class that's why they aren't
showing your custom fields. You can iterate all the items in the folder and
set the message class to your custom message class. BTW, 2 suggestions:
Don't use spaces in custom form message classes and always use the Version
property and increment it every time you make a change to your custom form.

under Personal Forms Library and I have saved it as "Message Classes". I
have tried clearing the forms cache, but as far as I know it didn't do
anything. I did this by going to tools, options, other, advanced options,
custom forms, manage forms, and then clear cache. Do I need to choose a
form to clear?
Do you have any other advice as to what I can do to make the information
from my custom form appear for all of my contacts?!
