Outlook crashes when using OL Project task Form

We are running 2003PS + Outlook2003.

Outlook add-in is loaded but whenever a user attempts to update their
tasks from within the Outlook form outlook crashes:

AppName: outlook.exe AppVer: 11.08118.0
ModeName: outllib.dll ModVer: 11.08118.0
fDebug: 0 Offset: 0000355f

Anyone come across this before?
