Outlook custom form - installing and accessing

Hi,

I was looking for a "while you were out" form to use in Outlook. I found a
couple and simiply copied them to the template location and I can get to the
form from the new menu, but you need to go pretty deep into the menus to open
a new form. For most users, this much to long to get a new form especially
for a phone message.

Is there a way to add this to the"new" list or to adda new item button on a
toolbar?
