|| Did you uninstall any add-ins for Outlook? Look in
|| Tools->Options->Other->Advanced->Add-in Manager (or COM add-ins) to
|| see what is there that you no longer have. If you don't see
|| anything that you can identify, then close outlook, rename
|| extend.dat to .old and reopen Outlook.
