Outlook form not displaying with 2003

I have a form that was working perfectly with NT. We have just upgraded to
2003 and the form can be seen by the person completing it but when it is
sent, the recipient can only see a blank page.

Anyone have any ideas?

My problem as well. Maybe the thread "Combining P2 on the Message page" dated
1/4 will help you.
