Outlook form not visible once sent outside of exchange server.

I have created an outlook to be used as a sort of notification email. Once we
email it, the recipient can not see it. It's blank.

That's expected behavior, especially if the recipient has Outlook 2003 SP2. For custom message forms to work, both sender and recipient need access to the published form definition, using the same MessageClass.

The recipient is using outlook 2000 same as I am. What do I need to do to get
the definition to them?

Send them the form as an .oft form template file with instructions to publish with the right message class to the Organizational Forms library or every person's Personal Forms library. Also see http://www.outlookcode.com/d/sendform.htm
