Outlook Form/Script

Hello everyone,
Im hoping someone on this forum can help me out. Ive had a lot o
experience working with Excel, Vbscripts, Access, and HTML, but don’
spend much time in Outlook. Let me try to explain what Im trying t
do…
I want to make a form that looks similar to this, not sure if it can b
done in Outlook or what….

Then I want Outlook to take the data from my fields and send it in
format like this

Anyways, if anyone has any ideas for me, or if its simply just no
possible, let me know.

To get you started go to Tools, Forms, Design a Form to allow you to amend
the inbuilt forms to accommodate your requirements. Also refer to
www.outlookcode.com.
