Outlook Form Submission

I was curious if anyone could help me with a small problem I have. I
have created a multipage message style form in outlook for the purpose
of using in an autoreply email account. The form has multiple pages of
info for the user to fill out & send back to the automated email.
Currently the only way it works is for the user to fill the info out on
the form, then forward the email & type in the address of the email to
forward to. I am looking for a way that the user can fill out the form
& just hit send so that the form will automatically be sent to where it
needs to go. Is there a code I can put somewhere to do this?

You could send the form as an .oft attachment preaddressed to a particular recipient.
