Outlook Form (survey) to show results in database

I am creating an Outlook Form (a survey) which will be posted in Publi
folders on our Exchange Server. I want everyone to fill out the for
and it automatically create a table within a database of thier results
Is this possible or am I asking too much. I already have the for
built but need to know how to link it to the database. Any help woul
be much appreciated. Thanks
