Outlook Forms Cache problem with new appointments

I have a client that has a re-occurring issue with the Outlook forms cache.
Whenever he tries to create a new appointment, he recieves an error that
Outlook cannot load the necessary form "an error occurred registering the
form in the OLE registry". We have read the related KB's and deleted the
cache both manually and using the tools menu. I have also tried just
manipulating the size of the forms cache. The next time Outlook is restarted,
the problem is back. Any help would be appreciated.
