Outlook Forms & Exchange 2003

Hi,
I'm not sure if this is the appropriate group to post this to but here
goes...

I'd like to find a form or template to use for my HR department to send out
for new hires. This form would include the new hires name, start date, a box
that would inform IT if the user needed a Laptop vs Desktop, etc.

I vaguely remember Microsoft having mad available some forms and templates
that did similar things that I may be able to modify for my needs.

Does anyone have any ideas on this?
