outlook forms in 2003

I've got a problem with the forms in outlook 2003.First we used the forms in
outlook 2000 and we had no problems,now we have outlook 2003 and the forms
don,t run automaticly and i also get some box in oulook saying some program
is trying to access e-mail addresses.... Can someone help me with this.
