Outlook Forms

I had created a form in Outlook, containing some specific fields. I had
received backup files (*.pst) egularly. Then, I had to reformat my PC. I
tried to reload my backup file. Unfortunately, I am able to see names,
addresses, and phone numbers. I can get the other fields that I had already
defined.

Would you please help me?

Either you have some typos in your post or it doesn't make sense. Could you describe the problem in more detail, please? And tell us where the form was originally located? And what you did to reload your backup .pst file? (And why you didn't reload the original instead of a backup.)
