Outlook Forms

I have created a form in OL 2002 and we are using Exchange 5.5 now the form
works except when I send it out it doesn't send the data that was inputted in
it, but it does send the form. I have published the form and even put it in
the public folder that I have owners permission on. could someone please help
me understand why my form isn't sending the data with it? thanks
