outlook hangs or freezes when using business contact manager

1ST Choice

When I attempt to link a phone log or appointment to a contact in BCM,
outlook totally freezes.
When I hit end task, all of Outlook shuts down and I get a message saying
that BCM has encountered a problem and needs to close.
Any suggestions how to fix this?
