Outlook/phone integration program?

Does anyone know of an application that will allow me to integrate use of
Outlook with my laptop's built in voice modem? For example, I'd like to plug
in a headset/microphone to the built-in sound card, select a contact in
Outlook to call, have the call operation start and have the call
automatically logged to the journal. Has anyone had any experience with a
program that will do that sort of thing?
