You might have success with Telephone Message Pad (
http://www.TelephoneMessagePad.com).
There are step-by step instructions for integrating with Outlook (
http://www.TelephoneMessagePad.com/outlook_tb.py). While the pictures are Outlook 2007, the same instructions work for Outlook 2003.

Our Company uses Outlook 2003...updating is not an option
I used an outlook form to advise recipient they had received a phone call.
It had a place for all the pertinent info and a text box to type in any
message left
My computer went haywire recently and the form was corrupted and ultimately
deleted by IT. I cannot find a similar form anywhere to replace this lost
form that I used daily. Thanks for any help you can provide

It works with Outlook 2003 as well.
It works with Outlook 2003 as well. You must connect the pst-file included
in the download to Outlook and access the form from there
