Outlook *sometimes* doesn't copy mail to sent items folder

I have a user that is having a problem with Outlook not saving mail to
his sent items folder, but only sometimes. The first time he reported
it today I looked things over, checked that he hadn't cleared the
option to save mail to the sent items folder. I searched all his
folders and could not find the message. As I was on the original
email I know it did get sent. I sent a few messages internally and to
my home email and they were copied to the sent items folder. I wrote
it off as a one time glitch.

Until it happened again. He's running Outlook 2003, SP1 on WinXP, SP1
(possibly SP2).

Any ideas?

does an Advanced Search locate those mails at all?
Any rules in place that might be acting on some sent emails?

Advanced search does not find them, he doesn't have any rules setup.
I think when it happens he has moved the message from his inbox
to a personal folder and replied from there.

Does he have the option "In folders other than the Inbox, save replies with
original messages" selected on Tools>Options>E-mail Options>Advanced E-mail
Options?

It looks like he was farwarding emails and didn't have the "Save Forwarded
Messages" option checked.
