Outlook toolbar

I am convertiang an app from an Exchange Extension to an Outlook add-in. I
have a button which only appears on certain form (Inbox and Sent items).
This is straightforward with Extensions but I cannot do it with Outlook
add-ins - It alwats seems to go onm the top level toolbar./
