Output Database Results or a Form to a Word Document

I have been tasked to put our Weekly reports online in a form so that we can
collect routinely data for analysis later on.
I want to put up either a form, or a database driven form such that the user
fills in the blanks clicks on save and then is able to choose
to send the output to Word, or not. Coulnd't find anything on the Microsoft
site, so thought I'd try here.
