Page Size vs Paper Size

How should a 4x6 in Landscape document be set-up?
Should I set the Paper Size first or the Page size?
Also, it is important to note that while I want the document to print in
lanscape mode, the paper must be fed into the printer in portrait orientation
because of its small size.

My printer is a HP1300
Publisher 2007

(I would've replied sooner but Microsoft was having some sort of problem
yesterday...

Really doesn't matter what printer as long as it can be setup with a custom
size.

File, Print Setup, create a 4 X 6 custom size. Or maybe you have an Index card
setting. My HP has a 4 X 6 index card selection.

In page setup, on the right, click advance, type 6 wide X 4 height. One page per
sheet.
