Password Database

Hi There,

I'm afraid that this is a really dumb question;

I need to set up a database to store records of System passwords. The
database needs to be secure (think I've mastered that bit), and it needs to
log details of the user who has entered the database to decode the password
(userid, time, date, etc). If possible, I also need the database to send
notification to the DB administrator when a password have been decoded.

I have tried several ways of doing this, but have finally hit a brick wall.

The database needs to hold details of around 60 systems, each with around 5
or 6 different passwords.

If there is anyone out then who can advise me on the best way to go about
setting this up I would be most greatful!
