password protected form won't open in switchboard

Hello.
I used the directions from article # 209871 and the form works the way I
want it to when I open it directly. The problem is that it won't open from
the switchboard. It says "calculating" at the bottom of the page and then
does nothing. I have minimized the switchboard to check and see if the popup
is running behind the SB and it is not. Does anyone have any ideas? If I
tweak the SB code I can get the form to open in DS view (the popup works) but
it won't work in form view - very confusing - Please help!
