Personalized template broken when run on 1 computer not others

I have a template form that allows all sorts of new commands in my file when
I attach it to the MS Word file. I open a document then attach the template
and then I get these new pull-down menus/buttons I can use for that file.

For some reason, the template has broken when I use it on one particular
computer. It lets me go through the attachment process and asks me to enable
macros (which is normal) but when the buttons and menus show up they don't
pull down and they are called Adobe PDF and Acrobat Comments. This is not an
Adobe template, but I think it somehow must have crossed with my Adobe
Acrobat Standard.

I've uninstalled and reinstalled both Acrobat and the template.
Note this isn't a problem on a different computer I have.

The PDFMaker addin supplied with Acrobat is a very bad neighbor. To sort out
your template, proceed as follows

1 Open Word.
2. Go to Tools, templates and add-ins, and uncheck the PDFMaker add-in
3. Delete any excess menus from your template
4. Save the template.

If you have included additional items in the main menu bar, it may be that
loading the PDFMaker add-in messes up your menu when it loads. If that is a
problem, post back again.

I don't have PDFMaker installed in the template list. I do have Endnote, but
because of what the menu names that install from my template become corrupted
to I don't think they are causing the problem. Is there a way to make sure
that PDF Maker is not interacting further?

Further information, if I open the .dot file directly, rather than opening a
..doc and attaching it, the .dot functionality (menus) are not corrupted. But
this does happen on any .doc file that I attach it to.
