Phantom "issues" appear on PWA Home Page and Issues & Risks page

Reposting from a few days ago:

A number of our users are seeing what I can only describe as
"phantom"
issues showing up in the Issue counter on the PWA Home Page and the
Risks & Issues page.

This is affecting just a few of our users, but those users who are
affected are all seeing the same thing. On their Home Page and on
their Risks & Issues page, they see a count of 7 issues. On the Risks
and Issues page, the spot where the project name should appear is
blank.

I have audited our workspaces both using Data Analysis and manually,
and I know for a fact that we have only one issue in the entire
system. Others have been created in the past, mostly in training and
testing, but all have been deleted except the one, legitimate open
issue.

Does anyone know:

1. How these issues are appearing on the issue counter of a small
subset of our users' screens, when I can find no other trace of them
anywhere in the system?

2. How do we get the affected users' issue counters to reflect
accurate info?

Have you deleted Projects from the Published database but left the
Workspaces? Orphaned workspaces (pre-SP1) can cause this behavior.

That was my first thought, and may have been the original cause.
However, any "orphaned" workspaces have since been deleted.

I have gone thru every workspace we have currently available, and can
find only one issue in one workspace (we use workspaces to retain
documents, but do not use the Issues feature at this time). I can see
an issue count of 7 in the Issues OLAP cube, in the Issues and Risks
counters on the PWA Home Page, and in the Issue counter on the Issues
and Risks page itself, but can see no other record of these issues
anywhere else.

I'm at a loss to explain this, nor can I fix it.

One other thought: the Issue and Risk counts displayed in the Project Center
and in the OLAP Cube are only update when the related project is
(re)published. Could this be old issues/risks, long since deleted (or
workspaces removed) where the project has not since been republished?

If that's the case (Issues and workspaces long since deleted), then
the projects have been deleted as well. What IS possible is that the
projects were deleted through "Delete Enterprise Objects", but the
"Delete the associates Windows SharePoint Services site" checkbox was
not checked. However, the workspace would have been deleted after the
fact from the "Project Workspaces" page in PWA.
