phone calls

How can I adequately record phone calls through Outlook?
These messages need to be sent to various contacts within my address book.
Currently I am having to type in the appointments section.

I.e. there used to be a message form that had fields for name, ph number,
re:, urgent/reply/action etc.

Is there any such message form in Outlook 03

Search for phone message template or something similar.
