phone formatting

Can someone please tell me how to format a text field that i have
combined other fields in so that the phone number has the following
format (999) 999-9999

design view =
=[First Name] & " " & [Last Name] & " PHONE:" & [PHONE] & " EXT:" &
[EXT] & " TOLL FREE:" & [TOLL FREE]

form view currently =
COLIN CHAN PHONE:4162461782 EXT:5513 TOLL FREE:
