Phone Log Form

I was trying to customize the Phone log form in Outlook w/BCM and now all I
get is a blank grey form, with no input fields. Is there a way to reload the
original form. Like a moron I saved the new form under the old name. HELP
IS GREATLY APPRECIATED.

I'm pretty sure that if you delete it from the Personal Forms library, then restart Outlook, the BCM addin will republish the form. If not, you can look in the BCM application folder for the .oft file.
