phone properties button disappears when moving field

hello,
I am using Outlook 2002 with SP3

I am designing a new form, but when I move any of the Business, Home,
Business Fax or Mobile field, where we should put the number, and I save my
form and open it in the normal mode (not in design mode) the pencil button
(that if you click on it, it gives a box to select country, area code and
type in number) that used to appear when you put your cursor in the fiels
will disappear ant wont show again, how to show that again? and if I want to
put a new phone number field, how to put that botton?
any help would be highly appreciated.
