Please suggest a template

I am a very beginner user of Excel and need help with a template suggestion.
I am using excel 2000. I need a simple spreadsheet with the following
information:

Employee's name
Class or training
Date of class or training
Instructor
Location

I would input all of our employee's names and update the list as continuing
education classes are completed by them. Any help is apprectiated. Mary

You don't need a template. Just use those headings in the first row as
headers and fill in as needed.
