Plugin does not load

It appears that I cannot add anything to the commandbar or toolbar with
a C# Outlook add-in.
Any ideas?

Need code to see what you are doing wrong here.

Have a look at ItemsCB (it's in VB6, but the idea is the same).
There are also some C# examples on MSDN.
