pop-up form as search

I would like to incorporate a pop-up form and enter search
criteria into designated fields and in return have a
report showing the requested infromation.
Ex. Say music search
Search1 Country/western
Search2 Slow
Search3 Bo James
If anyone can help I would really Appreciate it

If you are using Access 2000 or above, you can use Filter by Form to
accomplish your task. Otherwise Query by Form will do the trick. (Filter by
Form is like Query by Form but withot the query).
