Populating fields in a word document

I'm looking for a way for employees of my company to open templates for memos, letters, faxes then be prompted with dialog boxes to complete basic information (name, title, address, csz, phone, etc.). Then once this information is filled in through the dialog boxes it would automatically populate the template in the way that I've designed it?
