PopUp form

Here is what I have:

I have a tblSpecialNotices that has a CompanyID, company
Name, and Special Notes about the company and a form for
Sales, I would like to be able to, enter the sales form
and the Company ID from the Sales form matches the
CompanyId from the tblSpecialNotices, a Pop-Up for will
come up with the information located in the Special Notes
section of the Table.

Does anyone have any ideas on how this can be accomplished?
