Porting Outlook fields to Web queries

I've designed a customized contact form for our corporate
directory. We also have a Web site that pulls up the
associate bio for each associate in the company. I'm
trying to find a way to get the web query statement to
pull the Full Name or the First Name and Last Name fields
from the Outlook form so that the Web Address field will
point to the correct associates Web-based bio on the
company Intranet. Can anyone assist with this
seamingless easy yet elusive task?
